Current and former employees, including retirees, of the Louisiana House of Representatives may receive a copy of their 2025 Form 1095-C, Employer-Provided Health Insurance Offer and Coverage upon written request. This form reports information about your health coverage offer and any coverage provided by the Louisiana House of Representatives for calendar year 2025.

Requests must be made in writing (email or mail). Forms will be provided no later than the later of January 31, 2027, or 30 days after your request is received.
